3. Your spouse's drivers license number (if applicable).
4. The name and address of spouse's employer (if applicable).
5. The names, telephone numbers and addresses of TWO references who reside outside of your home.
6. The account number and the name of the lender of the loans you are consolidating.
7. Your official out-of-school date.
